Work Shift: Night from 6 pm to 3 am (for Daylight from 7 pm to 4 am)

Location: Shahrah e Faisal Karachi and Block A, Sector I-9/3, Islamabad.

GENERAL JOB DESCRIPTION:

The Clinical facilitator (SDOH screening) is responsible for conducting screening of individuals to assess social determinants of health. This role focuses on identifying social, economic and environmental conditions that impact patients such as access to healthcare, housing stability, food security, education and social support.

Clinical facilitator will also create referral for patient to connect them with appropriate community resources and ensure their social needs are addressed

KEY RESPONSIBILITIES:

  1. Conduct SDOH Screenings:
    • Use standardized tools to assess social determinants of health, including factors such as housing, food insecurity, employment, transportation, education, social support, and access to healthcare.
  2. Patient Outreach and Engagement:

    Reach out to patients proactively to conduct SDOH screening especially those at high risk due to identified social factors or prior healthcare utilization.

  3. Referrals initiation and completion:
    • Connect patients with appropriate community resources such as housing assistance, food pantries, transportation support, or social services.
    • Follow-up with the patient to ensure patients have access to necessary resources.
  4. Document and Track SDOH Data:
    • Accurately document screening results in DataQ health platform
    • Track progress of referral.
    • Ensure all subsequent screenings are performed.

SKILL SET & ABILITIES:

  • Strong communication skills (US English), both verbal and written with experience in patient education
  • Ability to educate patients and caregivers in a clear, empathetic manner
  • Proficiency with Electronic Health Records (EHR) and healthcare software
  • Strong organizational skills and attention to detail
  • Ability to manage multiple priorities and work independently
  • Knowledge of chronic disease management, medication management and common adherence barriers
